The Health and Wellbeing Department deliver a range of programmes to improve the health of our participants. We target those most in need and utilise community based, person-centred approaches to deliver excellent programmes. We work with a range of funders including Wandsworth Council, CCG’s, and charities such as Macmillan and Mind. We have developed considerably in the past few years and are looking to continue that growth in the future. This exciting post will be responsible for leading our Macmillan Community Social Prescribing and Personalised Care project which is delivered in partnership with Macmillan across Wandsworth and South West London. The postholder will be responsible for all aspects of the project, and building on the success achieved to date, continue to respond to the changing needs of people living with and beyond cancer. This role will explore opportunities for future funding and the long term sustainability of the project outcomes. We are looking for someone who is an experienced project manager with direct experience of managing health related projects, ideally in the cancer field. You will need to work across a wide range of project areas including budgets, staffing, referral pathways, delivery models, and monitoring and evaluation. You should be comfortable communicating with health and social care professionals, senior managers, operational teams, and the public, and able to quickly build effective relationships with all our stakeholders.
