
Live Well Community Referral is a groundbreaking new approach to supporting people who are experiencing barriers – such as anxiety, isolation, low confidence or mood – to reconnect with their communities and participate in local activities that can transform their health and wellbeing.
Support is tiered across three levels – Direct, Connect and Navigate – ranging from light touch to intensive support depending on participant need.
Live Well Advisers and Assistants have regular contact points building trust and relationships to help participants achieve their wellbeing goals by participating in community, cultural and physical activities.

This community initiative fosters healthier, happier, and more active communities in 33 Halo leisure facilities across nine regions. Volunteering is vital way to engage and build communities, connect with local influencers, and break down barriers to participation. ‘Be a Halo Hero’ expanded Halo Leisure’s active volunteer base from 17 to 113 (at the beginning of 2026), achieving an impressive 564% growth (+96 volunteers).
These volunteers support physical/social activity in gyms, pools, cycle tracks, and group classes, cafes, enabling essential community programmes like dementia-friendly sessions, autism-swimming and accessible cycling.
A core principle guiding this entire project has been the emphasis on human-interest narratives, focusing equally on the volunteers themselves and the individuals who benefit directly from their dedication.

Oldham Active Communities takes health, wellbeing and support directly into trusted community settings, helping residents overcome barriers such as poor health, isolation, poverty and low confidence.
Working alongside community groups, health services, local organisations and residents, the programme focuses on building trust, responding to local needs and creating pathways into physical activity, health support and wider community services.
By meeting people where they are and building strong local partnerships, Oldham Active Communities is helping to reduce health inequalities and create healthier, more connected communities across Oldham.

Pendle Leisure’s Creative Health Programme is a vibrant, multi-dimensional initiative designed to nurture healing, connection and personal growth through nature, creativity and community. At its heart is a belief in the power of hands-on, expressive experiences to transform lives.
Therapeutic horticulture and family-focused healthy eating and growing sessions are held in the community allotment, where participants reconnect with nature, grow their own food and share healthy meals. These sessions boost physical wellbeing, family bonding and mental health.
